Tooth decay is one of the most common microbial infections in the world. The World Health Organization (WHO) estimates that 60% to 90% of all school-age children suffer from dental caries . Dental caries is a multifactorial disease affecting children and adults that causes demineralization of the tooth structure. Major causative factors include the presence of endogenous cariogenic bacteria, frequent consumption of fermentable carbohydrates, and susceptible teeth and hosts.
Caries can develop if left untreated, but it can be prevented, reversed or stopped in its initial stages. Fluorinated varnishes have been shown to help deter and prevent dental caries.
Biomineralization is a complex dynamic process that occurs throughout life. Tooth demineralization is caused by acids in food and beverages and microbial attack by bacteria . These acids lead to a decrease in plaque pH followed by chemical dissolution of the organic and inorganic matrix and demineralization of the enamel. Phosphate and calcium ions diffuse into the oral cavity, leading to demineralization of the tooth enamel.
Topical fluoride can be used to enhance remineralization. Topical fluoride increases the availability of fluoride ions in saliva and promotes fluorapatite formation. Fluorapatite is highly resistant to acid attack and demineralization. In addition, increasing the concentration of fluoride may inhibit bacterial metabolism.
